Type 2 diabetes is a serious, chronic health condition that can lead to other serious health issues such as heart disease, stroke, adult blindness, lower-limb amputations, and kidney failure.

You can prevent or delay type 2 diabetes with proven, achievable lifestyle changes – such as losing a small amount of weight and getting more physically active – even if you are at high risk.

 

 

About the Hastings Diabetes on Track Coalition

Diabetes on Track is an initiative designed to improve diabetes care and prevention in rural Nebraska communities. The Hastings Diabetes on Track Coalition is made up of local organizations, government officials, professionals, and community members combining expertise and passion to help lower the rate of uncontrolled diabetes cases and to reduce the rate of diabetes progression among those with prediabetes.  The coalition is working to help people with diabetes get on a healthy track through food, physical activity and education. And, to create a community that helps people at risk for diabetes or those with prediabetes to stay on track to make healthful lifestyle choices and prevent diabetes.

This effort is made possible thanks to funding from The Diabetes Care Foundation of Nebraska to the University of Nebraska Foundation. The University of Nebraska Medical Center (UNMC) is working with the South Heartland District Health Department to support efforts in the Hastings community.
